There are 2 Forms required for all swimmers this season, your swimmer(s) can not practice or swim without these completed:

2 - Overlake/Midlakes League Participation Form and Code of Conduct Forms:  Every Overlake Family is required to have this form signed by a parent and Overlake is required to have this information on file for the duration of the season. Please click here for 2008 Overlake Swim team clothing and items order form.  Order forms have to be turned into pool office by Saturday 5/24 pool closing.  Please give to guard at pool desk.
